Dosing and injections 30 questions

Why do GLP-1 doses start low and go up?

GLP-1 doses start low and rise in steps, called titration, to give your gut time to adjust and reduce nausea and other stomach side effects. The first dose is a starting dose, not a maintenance dose.

For weekly injections such as Zepbound and Wegovy, the labels raise the dose every 4 weeks. Daily Saxenda rises weekly, and the Wegovy pill every 30 days. Map out your steps with the GLP-1 dose schedule calculator.

What is the Zepbound dosing schedule?

Per the Zepbound label, you start at 2.5 mg once weekly for 4 weeks, then increase in 2.5 mg steps after at least 4 weeks on each dose: 5 mg, 7.5 mg, 10 mg, 12.5 mg and 15 mg.

Maintenance doses are 5 mg, 10 mg or 15 mg weekly, and 10 mg or 15 mg for sleep apnea. The maximum is 15 mg. Your prescriber decides whether and when to step up. See Zepbound dosage.

What is the Wegovy dosing schedule?

Per the Wegovy label, you inject 0.25 mg weekly for weeks 1 to 4, 0.5 mg for weeks 5 to 8, 1 mg for weeks 9 to 12, 1.7 mg for weeks 13 to 16 and then 2.4 mg from week 17.

The usual maintenance dose is 2.4 mg, or 1.7 mg if 2.4 mg is not tolerated. If a step causes too many side effects, the label allows delaying the next increase by 4 weeks. Details on Wegovy dosage.

What is the Ozempic dosing schedule?

Per the Ozempic label, you start at 0.25 mg once weekly for 4 weeks, then move to 0.5 mg. If more blood sugar control is needed after at least 4 weeks, the dose can go to 1 mg, and after at least 4 more weeks, to 2 mg.

The 0.25 mg starting dose is not meant to control blood sugar on its own. Maintenance doses are 0.5 mg, 1 mg or 2 mg weekly. More on Ozempic dosage.

Is the Mounjaro dose the same as Zepbound?

Yes, Mounjaro and Zepbound use the same weekly dose strengths: 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg and 15 mg, with increases of 2.5 mg after at least 4 weeks on each dose.

The labels differ on maintenance. Mounjaro lists 5 mg to 15 mg for blood sugar control, while Zepbound lists 5 mg, 10 mg or 15 mg for weight. Your prescriber picks the target. Try the tirzepatide dose calculator.

Can I stay on a lower dose of Zepbound if it's working?

Possibly. The Zepbound label lists 5 mg, 10 mg and 15 mg as maintenance options, so moving up to the highest dose is not required for everyone. The 2.5 mg starting dose is not a maintenance dose.

Many prescribers hold a dose if weight loss is steady and side effects are manageable. Do not change your plan on your own. Talk with your prescriber about whether staying put fits your goals and health needs.

Can I increase my Wegovy dose faster?

No, the labels do not support moving up faster than the set schedule. For Wegovy and Zepbound, increases come after at least 4 weeks on the current dose.

Stepping up too fast raises the chance of nausea, vomiting and dehydration, which can lead to kidney problems. Weight loss is not much faster in the short term either, because the drug takes weeks to reach steady levels. If you feel your current dose is not working, discuss timing with your prescriber.

What if I miss a Zepbound or Mounjaro dose?

Per the Zepbound and Mounjaro labels, take the missed dose as soon as possible within 4 days (96 hours). If more than 4 days have passed, skip it and take the next dose on your usual day.

Do not take two doses to make up for a missed one. If you have missed several weeks, ask your prescriber whether to restart at a lower dose. See Zepbound missed dose.

What if I miss a Wegovy dose?

Per the Wegovy label, if your next scheduled dose is more than 2 days (48 hours) away, take the missed dose as soon as possible. If it is less than 2 days away, skip it and take the next dose on your usual day.

If you miss doses for more than 2 weeks, ask your prescriber how to restart, since starting back at a higher dose can bring stronger side effects. More on the Wegovy missed dose page.

What if I miss an Ozempic dose?

Per the Ozempic label, take the missed dose as soon as possible within 5 days of the missed dose. If more than 5 days have passed, skip it and take the next dose on your usual day.

Do not double up. After a longer gap, ask your prescriber whether to restart at a lower dose, especially if you had stomach side effects before. See the Ozempic missed dose page for examples.

What if I miss a Saxenda dose?

Per the Saxenda label, if you miss a daily dose, take the next one at the usual time the next day. Do not take an extra dose or increase the dose to make up for it.

If you miss more than 3 days in a row, ask your prescriber about restarting at 0.6 mg, since jumping back to 3 mg can cause strong stomach side effects. Victoza has a similar rule. Details on the Saxenda missed dose page.

What if I miss a Trulicity dose?

Per the Trulicity label, if your next scheduled dose is at least 3 days (72 hours) away, take the missed dose as soon as possible. If it is less than 3 days away, skip it and take the next dose on your usual day.

Never take two doses within 3 days of each other. If you are unsure how much time has passed, ask your pharmacist. See the Trulicity missed dose page.

How do I restart Wegovy after missing several weeks?

Ask your prescriber before restarting. The Wegovy label says that if you miss more than 2 weeks, you should check with your prescriber about how to restart, because your body may no longer be used to the higher dose.

Many people restart at a lower dose and step back up to reduce nausea and vomiting. The same caution applies to Zepbound and other GLP-1s after a long gap. Do not restart at your old dose on your own.

Can I change my weekly injection day?

Yes, if you keep enough time between doses. Per the Zepbound and Mounjaro labels, the last dose must be at least 3 days (72 hours) earlier. Per the Wegovy and Ozempic labels, there must be at least 2 days (48 hours) between doses. Trulicity also needs at least 3 days.

After the switch, stay on the new day each week. A phone reminder helps. Learn more on how to take Zepbound.

Where do you inject Zepbound or Wegovy?

Inject under the skin of your stomach, the front of your thigh or the back of your upper arm. Per the Zepbound instructions, stay at least 2 inches from your belly button, and an upper-arm shot is best done by someone else.

Rotate the exact spot every week, and avoid skin that is bruised, tender, scarred or hard. Read the Instructions for Use that came with your pen, since steps differ by brand.

Does injection site affect how well Ozempic works?

The labels list the stomach, thigh and upper arm as acceptable sites and do not tell you to prefer one over the others. Pick whichever you can reach comfortably and use correctly.

What matters more is good technique, rotating spots to avoid lumps under the skin, and injecting on schedule. If one area keeps bruising or getting irritated, try another site and mention it to your pharmacist.

How do you use a Wegovy or Zepbound pen?

Both are single-dose pens with a hidden needle. In general, you remove the cap, place the pen flat against clean skin, press the button as the Instructions for Use describe and hold it in place until the pen signals the dose is complete.

Each brand has its own clicks or indicator, so follow the leaflet for your pen and ask your pharmacist for a demonstration on your first fill. Never reuse a single-dose pen. See how to take Wegovy.

Do GLP-1 injections hurt?

Most people feel only a quick pinch or nothing at all, because the needle is short and thin and goes just under the skin. Mild stinging, redness or a small bruise afterward can happen.

Letting a refrigerated pen sit at room temperature briefly, if your Instructions for Use allow, relaxing the area and rotating sites can make shots more comfortable. Tell your pharmacist if pain is strong or lasting, as technique may need adjusting.

What is the difference between Zepbound pens and vials?

Zepbound comes as a single-dose pen with a built-in hidden needle, or as a single-dose vial that you draw up with a separate syringe and needle. The medicine is the same.

Vials are sold through LillyDirect self-pay pricing for certain doses and usually cost less, but they take more steps. Ask your pharmacist to show you how to draw up and inject a vial dose. See how to take Zepbound for both methods.

How many doses are in an Ozempic pen?

An Ozempic pen is a multi-dose pen that holds several weeks of doses, about 4 weeks for most strengths. You attach a new needle for each injection and dial the dose your prescriber set.

Check the flow before the first use of each new pen, never share a pen even with a new needle, and throw it away 56 days after first use even if medicine is left. The pen's Instructions for Use list doses for your strength.

How should I store my GLP-1 pens?

Most GLP-1 pens are stored in the refrigerator at 36°F to 46°F (2°C to 8°C) and must never be frozen. Each label allows some time at room temperature: up to 21 days for Zepbound, up to 28 days for unopened Wegovy and up to 56 days for Ozempic after first use.

Keep pens in the original carton to protect them from light. Tablets such as the Wegovy pill stay at room temperature. See Zepbound storage.

Is my Ozempic pen still good if I left it out of the fridge?

Probably, if it stayed within the label limits. Per the Ozempic label, after first use the pen can be kept at 59°F to 86°F (15°C to 30°C) or refrigerated for up to 56 days. For Zepbound, the limit is 21 days at or below 86°F, and for Wegovy, 28 days at 46°F to 86°F.

If it got hotter than 86°F, ask your pharmacist or the manufacturer before using it. More on Ozempic storage.

Can I use a GLP-1 pen that froze?

No. The Zepbound, Wegovy, Ozempic and other GLP-1 labels say not to use a pen or vial that has been frozen, even after it thaws. Freezing can damage the medicine.

Keep pens away from the back wall and freezer section of your refrigerator, where items can freeze, and do not pack them directly against ice packs when traveling. Ask your pharmacist about a replacement if a pen froze.

Can I fly with Ozempic or Zepbound pens?

Yes. TSA allows injectable medicines and their needles in carry-on bags. Tell the officer at the start of screening and keep pens in their labeled packaging to make inspection easier.

Keep pens in your carry-on rather than checked bags, which can get too cold or hot. An insulated pouch helps, but do not let pens touch ice directly. Check the room-temperature limit on your label before a long trip. See traveling with GLP-1 pens.

How do I take my weekly shot when traveling across time zones?

For weekly GLP-1 injections, the exact hour does not matter much. Take it on your usual day in local time, any time of day.

If a trip shifts your schedule, you can usually move your injection day, as long as you keep the minimum gap your label sets: at least 3 days for Zepbound, Mounjaro and Trulicity, and at least 2 days for Wegovy and Ozempic. For daily tablets, keep your morning routine on local time.

How do I dispose of Ozempic needles and pens?

Put used needles, pens and syringes in an FDA-cleared sharps disposal container right after use, and never throw loose needles in the trash or recycling. Stop filling the container when it is about three-quarters full.

Disposal rules vary by state and city. Options include pharmacy or hospital drop-off sites, household hazardous waste sites and mail-back programs. Your local trash service, health department or pharmacist can tell you what is allowed where you live.

What if I accidentally took two doses of my GLP-1?

Contact your prescriber or Poison Control at 1-800-222-1222 right away. A double dose can cause strong nausea, vomiting and, in people on insulin or a sulfonylurea, low blood sugar.

Because weekly GLP-1s last for days, symptoms may continue for a while. Sip fluids and watch for dehydration. If you have severe vomiting, confusion, fainting or signs of very low blood sugar, call 911. Bring the pen or packaging with you.

Can I switch from Ozempic or Wegovy to Zepbound?

Yes, switching is common, but your prescriber decides how. The labels do not include a conversion chart between semaglutide and tirzepatide, so the starting dose depends on your current dose, side effects and goals.

Usually you take the new drug on the day your next old dose would have been due, rather than overlapping. Never take two GLP-1 medicines together. If you need a prescriber to discuss switching, a clinician can reach out through find a clinician.

Can I switch from Wegovy shots to the Wegovy pill?

Possibly, but only with your prescriber's guidance. The Wegovy pill has its own dose steps, starting at 1.5 mg daily for 30 days, then 4 mg, 9 mg and a 25 mg maintenance dose, per its label.

Your prescriber will decide which tablet dose to start based on your injection history and side effects. Remember the pill needs strict empty-stomach timing every morning, which suits some routines better than others. See Wegovy vs the Wegovy pill.

How do you take the Wegovy pill or Rybelsus?

Take the tablet first thing in the morning on an empty stomach with a sip of plain water, no more than 4 ounces, and swallow it whole. Then wait at least 30 minutes before eating, drinking anything else or taking other oral medicines.

Food, coffee or extra water in that window can cut how much medicine is absorbed. If you miss a dose, skip it and take the next one the following day. See how to take the Wegovy pill.
