• The page address (URL)
• What looks wrong
• A source that shows the correct information, if you have one

What happens next

• We acknowledge your report within 2 business days.
• We check the claim against the primary source.
• If it's wrong, we fix it, update the page date and log the correction below. Health corrections go through clinician review.
• If we believe the page is right, we reply with the source we relied on.
