Does Jardiance cause weight loss

Jardiance makes your kidneys pass extra sugar, a few hundred calories' worth a day, out in your urine.

How much weight people lose

Yes, Jardiance often causes a small amount of weight loss. In diabetes trials, people on empagliflozin typically lost around 2 to 3 kg (about 4 to 7 lb) more than placebo. Part of the early drop is fluid. Results vary, and it isn't approved for weight loss.

Why it causes weight loss

Jardiance is an SGLT2 inhibitor. It blocks the kidneys from taking sugar back into the blood, so extra glucose leaves in the urine, roughly 60 to 80 grams a day in people with diabetes. That's a few hundred calories. Over time the body often compensates with a bit more appetite, so loss levels off.

The calorie math, worked out

Each gram of glucose carries about 4 calories. So 70 grams of sugar in the urine is about 280 calories a day. Over a week, that's close to 2,000 calories, which on paper could be about half a pound.

Real life is different. Many people start eating a little more without noticing, and less sugar is lost as blood sugar improves. That's why most people end up losing a few pounds in total, not a steady half pound a week. People without diabetes, whose blood sugar is lower, lose less glucose in the urine to begin with.

If you'd like to keep losing after the early drop, the usual tools still apply: a modest deficit from the calorie deficit calculator, enough protein and regular movement.

What it's approved for

  • Improving blood sugar in type 2 diabetes.
  • Lowering the risk of cardiovascular death in adults with type 2 diabetes and heart disease.
  • Heart failure and chronic kidney disease in certain adults.

The label lists 10 mg and 25 mg tablets taken once a day. Your prescriber chooses the dose based on why you take it and how your kidneys are working; for heart failure and kidney disease, the label dose is 10 mg.

What the research says

Jardiance's strongest evidence is for the heart and kidneys, not weight. Large outcome trials found fewer heart-related deaths in people with type 2 diabetes and heart disease, fewer heart failure hospital stays, and slower kidney decline in certain adults. The weight effect is real but small and fairly consistent across trials: a few kilograms, most of it early. Evidence grade for weight: moderate, because weight was a secondary result, not the main goal of the trials.

Side effects to know

  • Genital yeast infections and urinary tract infections.
  • Dehydration and dizziness, especially with diuretics.
  • Ketoacidosis, which is rare and can happen even when blood sugar isn't very high.
  • A rare serious genital-area infection (Fournier's gangrene).

Simple habits help: sip fluids through the day, stand up slowly if you feel lightheaded, and keep the genital area clean and dry. Ask your prescriber for a sick-day plan, since the label says to consider pausing it during illness, long fasting or surgery.

Who should be careful

  • People with low kidney function. The label limits its use for blood sugar control when kidney function is very low.
  • People with type 1 diabetes. It isn't approved for them, and the ketoacidosis risk is higher.
  • People on very low-carb diets or fasting. The label lists cutting calories sharply as a possible trigger for ketoacidosis.
  • People taking water pills or who are older, since dehydration and dizziness are more likely.
  • People on insulin or a sulfonylurea, who may have more low blood sugar.

Drink enough fluids through the day: find your target with the water calculator.

Jardiance vs weight-loss medicines

MedicineApproved for weightTypical average loss
JardianceNoabout 2 to 3 kg more than placebo
WegovyYes14.9% in STEP 1
ZepboundYesup to 20.9% in SURMOUNT-1

Medicines approved for weight produce far larger average losses. Some people with type 2 diabetes take an SGLT2 inhibitor and a GLP-1 together, since they work in different ways. That's a decision for your prescriber. Compare options in every approved weight-loss medicine.

When to talk to your doctor

  • If you want to use Jardiance for weight: it would be off-label.
  • If you have nausea, vomiting, belly pain and unusual tiredness (possible ketoacidosis).
  • If you have pain, redness or swelling in the genital area with fever.
  • Before surgery, as the label advises pausing it beforehand.
