The weight loss timeline
Weight loss on Farxiga is usually small and fastest early on. Most of it happens in the first few months, then it levels off.
| Period | What usually happens |
|---|---|
| First weeks | A quick small drop, partly fluid from passing more urine |
| First few months | Gradual loss as extra glucose leaves in the urine |
| After about 6 months | Loss tends to level off as appetite adjusts |
In trials, people on dapagliflozin typically lost about 2 to 3 kg (about 4 to 7 lb) more than placebo. Results vary.
A worked example at 210 lb
For someone who starts at 210 lb, 2 to 3 kg is about 4 to 7 lb, or roughly 2% to 3% of body weight. Some of the first few pounds may be water, which can come back if you stop the medicine. That's a real change, and small wins count, but it's a different scale from medicines approved for weight. Wegovy's trial average of 14.9% would be about 31 lb at 210 lb. See your own numbers with the weight-loss projection.
Why the scale stops moving
A plateau on Farxiga is normal and doesn't mean it stopped working for blood sugar, heart or kidneys. Three things usually explain it:
- Appetite tends to rise a little to make up for the calories lost in urine.
- As blood sugar improves, less sugar spills into the urine.
- The early fluid loss is a one-time drop.
If you want to keep losing, the usual tools still apply: a modest calorie deficit from the calorie deficit calculator, enough protein, and regular movement. More on plateaus: weight-loss plateaus.
Why it causes weight loss
Farxiga is an SGLT2 inhibitor, like Jardiance. It stops the kidneys from reabsorbing some glucose, so extra sugar and calories leave in the urine. The body often responds with slightly more appetite, which is why loss stays modest. Compare: Jardiance and weight loss.
What it's approved for
- Improving blood sugar in type 2 diabetes.
- Heart failure in certain adults.
- Chronic kidney disease in certain adults.
The label lists 5 mg and 10 mg tablets taken once a day; for heart failure and kidney disease, the label dose is 10 mg. Your prescriber sets the dose. Like Jardiance, its strongest evidence is for protecting the heart and kidneys, with weight loss as a side effect.
Farxiga vs Jardiance vs a GLP-1
| Farxiga | Jardiance | Wegovy | |
|---|---|---|---|
| Type | SGLT2 inhibitor | SGLT2 inhibitor | GLP-1 receptor agonist |
| How it's taken | Daily tablet | Daily tablet | Weekly shot |
| Approved for weight | No | No | Yes |
| Typical weight effect | about 2 to 3 kg vs placebo | about 2 to 3 kg vs placebo | 14.9% average in STEP 1 |
Some people with type 2 diabetes take an SGLT2 inhibitor and a GLP-1 together, since they work in different ways. That's a decision for your prescriber.
What the research says
The evidence for Farxiga is strongest for the heart and kidneys. Large outcome trials found fewer heart failure hospital stays and slower kidney decline in certain adults, including some without diabetes. Weight loss showed up consistently in the diabetes trials, but it was a secondary result and stayed modest. Evidence grade for weight: moderate. Evidence that it causes large or lasting weight loss: none so far.
If your main goal is weight, ask a licensed clinician whether you meet the criteria for a medicine approved for weight. Check your BMI first with the BMI calculator.
Side effects to know
- Genital yeast infections and urinary tract infections.
- Dehydration, low blood pressure and dizziness.
- Rare ketoacidosis, even with near-normal blood sugar.
- A rare serious genital-area infection (Fournier's gangrene).
Staying hydrated matters: find your water target.
Tips for the first few weeks
- Sip fluids through the day, more in hot weather or when you're active.
- Stand up slowly if you feel lightheaded, and tell your prescriber if it keeps happening.
- Gentle daily washing and dry, breathable underwear can help lower the risk of genital yeast infections.
- Ask your prescriber for a sick-day plan. The label says to consider pausing it during illness, long fasting or surgery, when ketoacidosis is more likely.
When to talk to your doctor
- If you want Farxiga for weight: it would be off-label.
- If you feel very weak, nauseous or short of breath (possible ketoacidosis).
- If you feel dizzy or faint when standing.
- Before surgery, as the label advises pausing it beforehand.
