What intermittent fasting is
Intermittent fasting limits when you eat. Common versions:
- 16:8: eat within 8 hours, fast 16
- 14:10: a gentler 10-hour window
- 5:2: eat normally 5 days, very little on 2
In trials, it tends to work about as well as ordinary calorie cutting, mainly by helping people eat less. Plan a window with the intermittent fasting calculator.
Why stacking the two needs care
The GLP-1 labels don't address fasting. But a GLP-1 plus a short window can push intake too low:
- Too little protein, raising the risk of muscle loss
- Too little fluid, raising the risk of dehydration
- Bigger meals crammed into a short window, which can worsen nausea
- Low blood sugar if you also take insulin or a sulfonylurea
A safer way to try it
| Step | What to do |
|---|---|
| Start wide | A 12-hour overnight fast, such as 7 pm to 7 am, before going shorter |
| Protect protein | Fit 3 protein-rich meals or 2 meals and a snack inside the window |
| Drink outside the window | Water and unsweetened tea or coffee are fine while fasting |
| Keep meals normal-sized | Don't make up for lost time with one huge meal |
| Check in | Stop if you feel faint, weak or your strength drops |

A sample 14:10 day
Here's one way a 10-hour window can fit around a small appetite while still reaching about 90 to 100 grams of protein:
| Time | What |
|---|---|
| 7:00 am | Water, black coffee or tea (fasting) |
| 9:00 am | Window opens: eggs and whole-grain toast |
| 12:30 pm | Chicken and bean salad |
| 3:30 pm | Greek yogurt with berries |
| 6:30 pm | Salmon, vegetables and potatoes |
| 7:00 pm | Window closes; water or herbal tea after |
This is an example, not a plan for everyone. If you can't fit enough food into the window, widen it.
Oral semaglutide and the morning
The label for Rybelsus says to take the tablet on an empty stomach with no more than 4 ounces of plain water, at least 30 minutes before the first food, drink or other oral medicines of the day. The Wegovy pill has similar instructions on its label. An overnight fast fits this routine naturally.
Who should skip it
- People with diabetes using insulin or sulfonylureas, unless their doctor plans for it
- Anyone with a history of eating disorders
- People who are pregnant or breastfeeding
- Anyone already struggling to eat enough on their GLP-1
When to call your doctor
- Shakiness, sweating, confusion or blurry vision, which could be low blood sugar
- Dizziness or fainting
- You regularly eat under about 1,000 calories without meaning to

Common questions
Can you do intermittent fasting on Ozempic or Zepbound?
The labels don't prohibit it, but combining a short eating window with reduced appetite can make it hard to eat enough protein and fluid. Talk with your doctor first.
Which fasting schedule works best with a GLP-1?
A gentle 12- to 14-hour overnight fast is easier to combine with a smaller appetite than a strict 16:8 or longer.
Does fasting make GLP-1 side effects worse?
It can. Cramming food into a short window often means larger meals, which can increase nausea and fullness.
