Why tirzepatide affects the pill
Tirzepatide slows stomach emptying, which can reduce how much of an oral medicine your body absorbs. The effect is strongest after the first dose and after a dose increase.
The Zepbound and Mounjaro labels advise people using oral hormonal birth control to switch to a non-oral method, or add a barrier method such as condoms, for 4 weeks after starting and for 4 weeks after each dose increase.
What about semaglutide
The labels for Wegovy and Ozempic don't carry the same 4-week backup instruction, based on the studies the FDA reviewed. Vomiting or diarrhea from any cause can still make pills less reliable, so ask your doctor about backup on sick days.
Planning a pregnancy
| Drug | What the label says |
|---|---|
| Wegovy, Ozempic | Stop at least 2 months before a planned pregnancy, because semaglutide takes a long time to leave the body |
| Zepbound, Mounjaro | Weight loss offers no benefit during pregnancy; stop when pregnancy is recognized and talk with your prescriber |
| Saxenda | Stop when pregnancy is recognized |
Labels change. Confirm the current wording on DailyMed and with your doctor.
Non-oral birth control options
Because the tirzepatide advice is about pills, a non-oral method avoids the absorption question entirely. Options to discuss with your clinician:
- Hormonal or copper IUD
- Implant in the upper arm
- Injection every 3 months
- Vaginal ring
- Skin patch
- Condoms, as a primary or backup method
Each has its own pros, cons and medical considerations, such as blood clot risk with some estrogen-containing methods. Your clinician can match one to your health history.
Weight loss itself can also change fertility. Losing weight may make ovulation more regular, particularly for people with polycystic ovary syndrome, so don't assume you are less likely to get pregnant than before.
If you get pregnant on a GLP-1
Tell your prescriber and obstetric provider right away. Animal studies raised concerns, and there isn't enough human data to know the risks well.
Drug makers run pregnancy registries to collect outcomes. Your provider can tell you how to enroll.
Breastfeeding
There is limited information on GLP-1 drugs in breast milk. The labels advise weighing the benefits of breastfeeding against the need for the drug, a decision to make with your doctor.
