The short answer
Obesity is treated as a chronic condition, and the FDA labels describe Zepbound and Wegovy for chronic weight management. In trials, most people regained a large share of their weight after stopping.
That doesn't mean everyone must stay on the drug forever. It means stopping is a decision to plan, not something to do quietly.
What the trials found
| Trial | Drug | What happened after stopping |
|---|---|---|
| STEP 1 extension | Semaglutide 2.4 mg | People had lost about 17% of body weight. One year after stopping, they had regained about two-thirds of that loss. |
| SURMOUNT-4 | Tirzepatide | After 36 weeks on the drug, people switched to placebo regained about 14% of body weight over the next year, while those who stayed on tirzepatide kept losing. |
Health markers that had improved, such as blood pressure and blood sugar, also tended to drift back toward where they started in the STEP 1 extension.

- After you eat, your gut releases a hormone called GLP-1. GLP-1 medicines copy it, but last much longer in the body.
- It slows how fast your stomach empties, so you feel full sooner and stay full for longer.
- It also acts on appetite centers in the brain, which quiets hunger and constant thoughts about food.
- When blood sugar is high, it helps the pancreas release insulin. Tirzepatide also acts on a second gut hormone, GIP.
Why weight comes back
When the drug leaves your system, appetite signals return to their old levels. Meanwhile your body, now smaller, burns fewer calories. That combination pushes weight up.
Semaglutide and tirzepatide are long-acting, so the effect fades over several weeks rather than overnight.
If you and your doctor decide to stop
Some people stop because of cost, side effects, pregnancy plans or coverage changes. Questions worth asking:
- Should I step down the dose or stop at once? Only your prescriber can decide this.
- How often should I weigh in for the first 6 months?
- At what amount of regain should we talk again?
- Do any of my other medicines need adjusting, especially for diabetes or blood pressure?
Use the weigh-in tracker so you spot regain early, and recheck calorie needs with the TDEE calculator.
Restarting after a break
Coverage gaps, shortages and surgery mean many people pause and restart. The labels include missed-dose rules, and some describe what to do after a longer gap. For example, as of our last review the Wegovy label allows restarting the dose escalation when more than 2 consecutive weekly doses are missed.
Going straight back to a high dose after weeks off can bring back strong nausea and vomiting, because your gut has lost its tolerance. That's why prescribers often restart at a lower step.
- Don't restart at your old dose on your own after a long gap
- Tell your prescriber exactly when your last dose was
- Expect side effects similar to when you first started
Check the missed-dose pages for Zepbound, Wegovy and Ozempic, or map the steps again in the dose schedule tool.
Habits that help after stopping
- Keep protein high and strength training going to hold on to muscle
- Keep meal structure: regular meals, planned snacks, smaller plates
- Stay active; people who maintain weight loss tend to move a lot
- Watch the trend, not the daily number
If cost is the reason you're stopping, see the GLP-1 cost index and prior authorization guide first. There may be options you haven't tried.
When to call your doctor
- Before you stop or skip doses for any planned reason
- If you have diabetes and your blood sugar rises after stopping
- If you regain weight quickly or your blood pressure climbs

Common questions
Will I gain the weight back if I stop Wegovy or Zepbound?
In trials, most people regained a large share. In the STEP 1 extension, about two-thirds of lost weight returned within a year of stopping semaglutide.
Can I taper off a GLP-1?
The labels don't describe a tapering schedule for weight management. Any plan to stop or step down should come from your prescriber.
How long does semaglutide stay in your body?
Semaglutide has a half-life of about a week, so it takes roughly 5 to 7 weeks to clear. Appetite effects fade over that period.
