Early on, a little loss is possible
Prozac (fluoxetine) is an SSRI. In the first weeks, some people notice a smaller appetite or mild nausea, and the scale can dip a little.
That early effect is the reason Prozac gets talked about for weight. It's not a weight-loss medicine, and it isn't approved for that use.
Why Prozac acts differently at first
Fluoxetine stays in the body much longer than most antidepressants. Its active breakdown product, norfluoxetine, can take weeks to reach steady levels. That's one reason its side effects, including a lighter appetite, can shift over the first month or two.
It's also why stopping Prozac tends to feel gentler than stopping shorter-acting SSRIs. Even so, any change should go through your prescriber.
Prozac and binge eating
Fluoxetine's approval for bulimia nervosa is about reducing binge-purge episodes, not weight. If eating feels out of control, that's worth raising directly; see emotional eating for gentle first steps.
With a GLP-1 medicine
Many people take both. Early nausea from each can overlap, so tell your prescriber about every medicine you use and keep fluids up.
Over the long run
With longer use, the appetite effect tends to wear off. Many people end up weight-neutral, and some gain a little as mood improves and eating returns to normal.
Fluoxetine is FDA-approved for several conditions, including depression, OCD, panic disorder and bulimia nervosa. In bulimia it's used to reduce binge-purge cycles, not to lower weight.
If you notice gain over months, look at the whole picture: sleep, activity, stress, other medicines and how your eating has changed. A steady, modest approach usually works better than a big overhaul.
What the research says
Prozac is one of the few antidepressants that has been studied directly for weight, so there's useful history here:
- Older obesity trials (moderate evidence). In studies from the 1990s, people with obesity given fluoxetine lost weight over the first few months. Much of it came back by 6 to 12 months, even while they kept taking it
- Recent health-record studies (moderate evidence). A large 2024 US study found weight change on fluoxetine over 6 months was similar to sertraline, and both were linked to less gain than options like escitalopram or paroxetine
- Individual results (wide range). Some people lose a little, many stay level and some gain
That's why experts don't treat Prozac as a weight tool. The early dip usually doesn't last.
What actually helps with weight
If weight is on your mind, the reliable levers are the boring ones: a modest calorie deficit, enough protein, strength training and sleep. For people who qualify, medicines designed for weight management do far more; see our medication roundup.
A realistic pace is about 0.5 to 2 lb a week. Check your starting point with the TDEE calculator, then choose a deficit you can live with.
A gentle way to track it
If you're curious whether Prozac is affecting your weight, collect a little data before drawing conclusions:
- Weigh in 2 to 3 mornings a week, at the same time, after using the bathroom
- Log the numbers in the weigh-in tracker and watch the weekly average
- Write a 1-line note on appetite and mood each week
- After 8 to 12 weeks, look at the trend with your prescriber
For example, someone at 170 lb might dip to 168 in the first month, then drift back to 169 or 170 by month 3. That pattern is common. To check daily energy needs, see the estimate for a 170 lb woman or a 170 lb man.
Common mistakes to avoid
- Expecting the early dip to continue. It usually levels off, and that's normal
- Skipping meals because appetite is low. Small regular meals with protein help protect muscle and energy
- Hoping a higher dose will help with weight. Dose decisions are about your symptoms, and your prescriber makes them
- Making changes on your own. Even with its long half-life, any change to Prozac should go through your prescriber
When to talk to your doctor
- You notice weight changes you didn't expect, up or down, after starting or changing a dose
- You want to switch or stop an antidepressant. Stopping suddenly can cause withdrawal-like symptoms
- Your mood gets worse, or you have thoughts of harming yourself. Call or text 988 any time
- You're considering a weight-loss medicine alongside your current prescriptions
