What compounded semaglutide is

Compounded drugs are made by pharmacies, not approved by the FDA, and not checked by the FDA for safety, quality or effectiveness before sale. During the shortage, compounding of semaglutide was widely allowed.

What changed

The FDA declared the semaglutide shortage over (semaglutide in February 2025). After short transition periods, mass compounding of copies of the approved drug was restricted. Compounding for an individual patient's documented medical need may still be allowed under federal law. Rules and enforcement keep shifting, so check FDA.gov for what is in effect.

Risks the FDA has flagged

  • Dosing errors, especially with vials measured in syringe units
  • Salt forms (like sodium or acetate) that differ from the approved drug
  • Products sold without a prescription or from unlicensed sellers
  • Reports of people hospitalized after measuring doses

Lower-cost approved options

Maker self-pay programs now sell approved semaglutide at lower cash prices than list. Compare them in the GLP-1 cost index.

Common questions

Is compounded semaglutide FDA-approved?

No. Compounded drugs aren't FDA-approved, and the FDA doesn't check them for safety or quality before sale.

Is compounded semaglutide still legal?

Mass compounding of copies was restricted after the shortage ended. Limited compounding for individual medical need may still be allowed. Check FDA.gov for current rules.
