Medicare and GLP-1 drugs

Medicare Part D has been barred by law from covering drugs used only for weight loss, but the same drug can be covered for a different approved use.

The rule behind the confusion

The law that created Medicare Part D excludes drugs used for weight loss. That's why a plan may refuse Wegovy for weight management but cover it for heart risk reduction.

Coverage depends on the reason the drug is prescribed, matched to an FDA-approved use.

How coverage has worked by use

DrugUsePart D can cover?
Ozempic, Mounjaro, Trulicity, RybelsusType 2 diabetesYes, if on your plan's formulary
WegovyReducing heart attack and stroke risk in people with heart disease and obesity or overweightYes, if on the formulary
ZepboundModerate to severe obstructive sleep apnea with obesityYes, if on the formulary
Wegovy, Zepbound, SaxendaWeight management aloneNot under the standard exclusion

Plans can still require prior authorization and proof of the diagnosis.

What changed in 2025 and 2026

In late 2025, federal announcements described agreements with drug makers to lower GLP-1 prices and widen access for some Medicare enrollees starting in 2026. Details, timing and eligibility were still being worked out at our last review.

Because this area is moving fast, check Medicare.gov for what is in effect when you read this, and call your plan to confirm.

Manufacturer savings cards and Medicare

Federal anti-kickback rules generally bar manufacturer copay savings cards from being used with Medicare, Medicaid or other government insurance. The card terms say this in the fine print.

Manufacturer cash-pay programs are a separate thing. Compare options in the cost index.

Medicaid and Medicare Advantage

Medicaid works differently. Each state decides whether its program covers drugs for weight management, and the answer varies a lot from state to state and can change with state budgets. Your state Medicaid office or managed care plan can tell you what is covered and what prior authorization is needed.

Medicare Advantage plans that include drug coverage follow the same Part D rules for prescription drugs. Some offer extra benefits such as nutrition or fitness programs, but these don't override the Part D exclusion for weight-loss drugs.

People with both Medicare and Medicaid, called dual eligibles, get their drug coverage through Medicare Part D, so the Medicare rules apply.

How to check your own plan

  1. Use the Medicare Plan Finder and enter your drugs and doses
  2. Look at your plan's formulary for the drug, tier and any PA or step therapy notes
  3. Ask your prescriber which diagnosis the prescription is written for
  4. During Open Enrollment, October 15 to December 7, compare plans for next year

Medicare Part D also has an annual out-of-pocket cap, which can limit what you pay for covered drugs. Check Medicare.gov for the current amount.

When to call your doctor

  • To ask whether you have a diagnosis that matches a covered use
  • If your plan denies coverage and you want to appeal
  • Before switching drugs to fit coverage, since doses don't convert one to one

Common questions

Does Medicare cover Wegovy?

Part D plans can cover Wegovy when it's prescribed to reduce cardiovascular risk in people with heart disease and obesity or overweight. Coverage for weight loss alone has been excluded by law.

Does Medicare cover Zepbound for sleep apnea?

Part D plans can cover Zepbound for moderate to severe obstructive sleep apnea in adults with obesity, if it's on the plan's formulary.

Can I use a Zepbound or Wegovy savings card with Medicare?

Generally no. Manufacturer copay cards usually exclude people with Medicare or other government insurance.

Did Medicare start covering GLP-1s for weight loss in 2026?

Federal announcements in late 2025 described expanded access for some enrollees. Check Medicare.gov for what is in effect now, because details were still changing at our last review.
